Application forms<\/h1>\n\n\n

1. Select a request (application) form that corresponds to your matter.<\/p>\n\n\n\n

2. Address your request (application) form to the appropriate deputy dean:
– if you are a full-time student:
to\u00a0dr hab. Natalia Paprocka, prof. UWr<\/strong>,
– if you are a part-time student:
to\u00a0dr hab<\/strong>.\u00a0Mateusz \u015awietlicki<\/strong>, prof. UWr.<\/strong><\/p>\n\n\n\n

3. Complete the form and prepare the necessary documents (e.g., IOS schedule, medical certificate etc.).<\/p>\n\n\n\n

4. In the footer of the form, check the document circulation path (e.g., student -> relevant head -> Dean\u2019s Office).<\/p>\n\n\n\n

5. If you need the opinion of the instructor or the relevant head (check who is such a person in your institute\/chair<\/a>), you can get it by e-mail.<\/p>\n\n\n\n

6. Submit the completed and signed form and all accompanying documents either by e-mail (as a scan) or in person<\/u>, following the document circulation path, to the relevant head or to the relevant employee of the Dean\u2019s Office <\/strong>(check who is such a person in the Dean\u2019s Office<\/a>).<\/strong><\/p>\n<\/div><\/div>\n<\/div><\/div>\n\n\n\n
